Release of Liability: Required for all Players

For the upcoming Season, all parents must complete a "Release of Liability" form before their player(s) participate. This is needed for all players across Tee Ball, Machine Pitch, Baseball, and Softball.  We are making it easy to complete by offering your acceptance of the waiver using the online form below.  Click the Step 1 button below to read the "Release of Liability" form. Then move to step 2 below to complete your waiver.
